We do recommend you enroll for at least 12 months with us but understand that life happens. We just ask that you provide us with a 30-day written notice to cancel your membership.

How do I cancel my membership?

Due to changes in federal and state legislation regarding narcotic pain medications, we do not routinely prescribe narcotics or manage chronic pain. A pain management specialist may collaborate with us as a multidisciplinary team to help provide you with the best care.

Do you prescribe chronic pain medications (opioids) and benzodiazepenes?

While we don't perform pap smears in-house, we are happy to refer you to a local gynecologist for your well-woman exams.

Our goal is to address all your primary care needs in-house. However, if your condition requires specialized attention, we're happy to help coordinate referrals on your behalf. 

What if I need a specialist? 

Our primary goal is to keep you healthy and avoid hospital visits. However, if you ever need emergency or inpatient care, we'll stay in touch with your hospital team and help coordinate your treatment. Once you're back home, we'll follow up to support your full recovery and return to your normal health and activities.

What if I need to go to the emergency room or get admitted to the hospital? 

In the event of an emergency, call 911. Our members are provided with several methods to contact us for urgent/non-urgent concerns. We are available for urgent situations that cannot wait until business hours the following day. In order for us to provide the best care to our patients, please only contact us after hours if it is urgent. 

Is there someone I can speak to for emergencies when the office is closed?

While we're not available 24/7, we do offer support beyond regular office hours for urgent matters. Our aim is to provide comprehensive outpatient care and help you avoid unnecessary trips to Urgent Care or the Emergency Department. For non-urgent issues, please contact us during office hours. We have specific guidelines for after-hours and emergency calls to ensure you receive appropriate care when you need it most.

Can I contact you at any time?

To ensure the best service for our patients and to allow ample time to address all their concerns, we operate on a scheduled appointment basis and do not accept walk-ins.

Once you're an established member, booking an appointment is easy! Just reach out to us via text, email or phone, and our team will gladly help you find a suitable time slot.

How do I make an appointment?

The goal is to keep you healthy and prevent illness. If you need a urine dipstick, antibiotics or have questions for your doctors, we will be there for you. Urgent care visits could cost as much as an annual membership. Our members enjoy the convenience of calling/texting/emailing us for easy care.

Will I benefit from direct primary care if I don't require frequent medical attention?

Medications, labs/bloodwork and imaging are offered at a discount, but not included in the membership. Our specialty services (Weight Loss, IV Hydration, Red Light Therapy, and Body Composition Scans) are available for an additional cost.

What is NOT included in the primary care membership?

You simply start by selecting your membership tier by going here and clicking "Enroll now" once you've made your selection. Once we receive your enrollment, you will then be prompted to schedule a free meet and greet. You will not be charged for this visit. This meeting is not to provide medical advice but more a chance for you to ask questions about the our membership model to see if we will be a good fit for you. If you decide to proceed, we will then email you new patient paperwork and get you scheduled for your first in-person appointment to establish care.  Your are not considered a member until you are seen for your first visit. Additionally, you will not be charged until you are actually seen for your first in-person visit. 

How do I become a member?

All of our services require a primary care membership except for our weight loss programs, health coaching programs, ADHD evaluations and Body Composition Scans. Our members receive priority appointment booking for all services.

Do I have to be a member to be seen?

Our fees are all transparent. No hidden co-pays, co-insurance, or deductibles for your primary medical care with us. Annual Lab tests are included only in the Signature Membership Plan. If there is an extra charge for a service, you will know exactly what it is before proceeding. Imaging and routine labs are not included in your membership. We have collaborated with Labcorp and TRA Imaging to ensure accessible and affordable pricing for these services.

What are the membership costs and are different plans available?

We do not bill insurance or Medicare for any of our services. Direct primary services are covered by your monthly membership. Insurance can still be utilized outside of our practice for visits with specialists and for imaging, labs, medications, and hospitalizations. We recommend that patients carry insurance to provide coverage for catastrophic events and other unexpected health needs.
